Conor Murray will captain the Lions for the first time in Wednesday’s clash with South Africa 'A’ in Cape Town.
Ireland team-mates Bundee Aki and Iain Henderson also named in the starting 15, while Tadhg Beirne is named among the replacements.
Murray, who came off the bench last Saturday, will partner Dan Biggar at half-back.
The South Africa 'A' side contains seven players that started in their 2019 World Cup final win.
Five of those are in the back-line with full back Willie Le Roux and Cheslin Kolbe getting a start in the Lions warm-up game in Cape Town.
There's also the centre pair of Damien De Allende and skipper Lukhanyo Am as well as scrum half Faf de Klerk.
Second row Eben Etzebeth and flanker Pieter-Steph du Toit will also start.
Leinster hooker Ronan Kelleher could be set for a late call-up to the Lions squad.
Head coach Warren Gatland says they are looking at cover in their front row positions.
Kelleher scored four tries for Ireland on Saturday in a comfortable win over the United States.
Meanwhile, Gatland claims it’s possible Alun Wyn Jones could rejoin the squad having recovered from a dislocated shoulder.
