Liverpool have called on European football's governing body to ensure there are no more "near misses", following an independent report into chaos at last year's Champions League final.

UEFA and French authorities have been blamed for failures which almost led to a "mass fatality catastrophe".

Sports journalist Daniel Austin was covering Liverpool's game with Real Madrid in Paris.

He's worried lessons won't be learned quickly enough for the upcoming rugby union World Cup, Olympics and Paralympics.
